All clients must fill out our intake form and submit the required documentation before making an appointment. If you need assistance with the form, please call our office to set up an intake orientation.
1. Press the "Intake Form" button below to link you to our online intake form. When linked to the intake, choose between two options:
* "Homeowner experiencing a financial hardship" if your hardship is NOT Covid-19 related.
*If your hardship began on or after March 1, 2020, due to COVID-19, choose the option "Homeowner experiencing a financial hardship since March 1, 2020, due to COVID-19."
2. Download the intake form, fill it out, and turn it in. You can either fax, email or drop off the application, along with all documentation, directly at our office.
